  • Complete and Up-to-date Immunization Record
  • Health Assessment for Kindergarten (must be completed by a physician)

 ** Children must show proof of the above by the 30th calendar day after school enrollment. After the 30th day, the principal shall not allow the child to attend school until proof of immunizations and/or the health assessment are provided. (See School Nurse for special circumstances.)

 

Immunization Clinic:

Immunizations are available from your child’s physician or Catawba County Public Health. For an appointment at Catawba County Public Health call 695-5881.

 

Does your child have a Medical/Dental Home:

Medical Home is the place you can take your child for checkups, sick visits, accidents, special health needs, and immunizations. You can call your Medical Home during office hours and after-hours. Contact your school nurse for more information on Medical Homes.

 

Does your child have health insurance?

Your child may qualify for free or low cost health insurance for children under the age of 19.  Contact your school nurse for more information.
